The North DeKalb Health Center is now offering sports physicals for student and non-student athletes. This service is provided by board certified Family Nurse Practitioners. Physicals are available Tuesday and Wednesday by appointment only. To schedule an appointment, please call (770) 454-1144. The cost of the physicals is $29.00 and payments must be made by credit or debit card. The North DeKalb Health Center is located at 3807 Clairmont Rd., NE, Chamblee, GA 30341.
