Registration Information

New Registration Requirements have gone into effect for the 2014-2015 school year.

Requirements for registration:

Parents or legal guardians MUST accompany a student to register
2 forms of  Proof of Residence in the Chamblee Charter HS attendance area. (lease, mortage, deed, utlity bill, etc.)
Birth certificate listing both parents
Withdrawal papers from previous DeKalb school
Eye, Ear, Dental Form (GA form #3300)
Immunization (GA form #3231) See new requirements below.
Social security card or signed waiver
Transcript and/or transfer grades and previous schedule

Immunization

Effective July 1, 2014, all children born on or after January 1, 2001 who are attending seventh grade and children who are new entrants into a Georgia school in grades eight through twelve, must have received one dose of Tdap vaccine and one dose of meningococcal conjugate vaccine. Please contact the DeKalb County Schools Student Health Department if there are questions.

A student shall not receive provisional enrollment while awaiting evidence of immunization requirements. Georgia law, specifically O.C.G.A. § 20-2-771(b), requires a certificate of immunization prior to enrollment. The law permits a principal to grant a thirty (30) day waiver of this requirement to a student who moves from an out-of-state school to DCSD school only.

All students registering at Chamblee Charter High School for the 2014-2015 school year must submit a current certificate of immunization form (Form 3231) prior to enrollment for the new year. You may obtain this information from your Primary Care Physician or a Dekalb County Health Department site.

Social Security Numbers:

A Social Security number is required for each student. However, providing the Social Security number is voluntary and a waiver for a parent/guardian to sign stating a preference not to provide the Social Security number should be presented. Please be aware that no student will be denied enrollment because the parent declined to provide a Social Security Number or declined to apply for a Social Security Number. However failure to supply a Social Security number will prevent your student from being eligible for Georgia's HOPE scholarship program in their senior year of high school.
